MSW Application Checklist

Note: Students applying to the Combined MSW/Ph.D. program should use the PhD application checklist

The application process for the UCLA Department of Social Welfare is comprised of two parts.Part One is the submission of basic information to the UCLA Graduate Division, including your academic history and the degree program to which you want to apply, as well as the $70 application fee for domestic applicants and $90 for international applicants.. Part Two of the application process is the submission of the following items directly to our department: Department of Social Welfare online application, personal and professional statements, resume, supplementary information, letters of recommendation, transcripts, and GRE scores.

For MSW applicants, the final deadline for filing both applications is January 15. 
For students applying for merit-based fellowships, the final deadline for filing ALL 3 applicaitons is December 15.

n All materials must be received or postmarked by the deadline
in one single packet. (GRE scores are sent separately by the testing center)

n Both the UCLA and Social Welfare Department Online Application
must be completed by the deadline.

n
Incomplete or late applications received after the deadline
will not be reviewed.

While the department can recommend whether a student should be offered admission, the Graduate Division has final authority in all admission decisions.

Application Checklist: PART ONE Graduate Division Application

Apply to the Graduate Division online

1) UCLA Application for Graduate Admission 2008 - 2009
Complete the Graduate Division application. Go to the Graduate Division Web site and click on the Online Application for Graduate Admission. Follow the instructions. You do not have to complete it all at one time. You can save the application and submit it when you have all the necessary information. Once you start the application you will be assigned a PIN # for retrieval and verification.

To insure that your application materials are properly routed, it is important that you use the correct UCLA major code (864) on your application to the Graduate Division and the correct GRE institution (R4837) and department (5001) codes on your GRE registration materials. If necessary, the TOEL institution code for UCLA is 4837 and the department code is 95.

2) $70/$90 Application Fee
The nonrefundable application fee ($70.00 for U.S. citizens and Permanent Residents; $90.00 for all other applicants) must be paid before an application can be processed. The UCLA Graduate Division accepts Visa, Mastercard, and Discover or, if necessary, check or money order payable to the UC Regents, addressed to: Remittance Processing Center 10920 Wilshire Blvd.Suite 107 Los Angeles, CA 90024
Please be aware that if you select the check option, the initial processing of your application will be delayed by a minimum of seven to 10 working days. The date your payment is received is considered date your application is submitted.

Please Note: The Social Welfare Department cannot waive or defer the application fee.

Application Checklist: PART TWO Department of Social Welfare Application
All supporting documents must be received or postmarked by the deadline. Incomplete or late applications will not be reviewed. (GRE scores are sent separately by the testing center)

Apply to the Department of Social Welfare. Send the following materials to:

1) Completed Social Welfare supplemental application online.

2) Two official copies of transcripts from each college or university attended in the United States or abroad (Include both undergraduate and graduate academic records, if applicable.)

3) Official copy of scores from the Graduate Record Examination (GRE) earned within the last five years. General Test only. (Sent separately by testing center)

4) Test of English as a Foreign Language (TOEFL) OR International English Language Testing System (IELTS) scores, if the applicant is a student whose native language is not English. (The TOEFL or IELTS) is required of all non-native speakers unless they have completed two years of university-level coursework in an English-language institution.)

5) Resume or chronological list (starting with the most recent) of all relevant employment since high school, including summer jobs and internships you consider significant. Please also include all relevant volunteer or other experience with service-oriented groups. For each job or volunteer experience, please include your title, the number of months and/or years you held the position, and the number of hours worked each week.

6) Three letters of recommendation from individuals familiar with the student’s academic and scholarly work. At least one of the references must be from a professor who has taught the student. Letters are to be submitted in a sealed envelope with a completed application packet. Please use the UCLA Letter of Recommendation Form.

7) Personal Statement should respond to the following:

    a) Describe the significant relationships and life experiences that have influenced your decision to seek a career in a helping profession.

    b) What values affected your decision to
    enter the social work profession?
    How are
    these values reflected in social work practice?

    c) Describe your experiences with diverse populations and groups, and how those experiences have contributed to your interest in social work. Which population (defined by culture, ethnicity, sexual orientation, socio-economic status, and psychological and physical functioning) would you like to serve?

    d)
    What are your expectations of graduate education in terms of your own development? Indicate any problem or limitations that should be taken into account in planning your graduate program.

Note: The statement should not exceed two double-spaced pages

8) Professional Concepts and Goals Statement should respond to the following:

    a) Describe your concept of social work as a profession and of the spectrum of clientele toward whom social work is oriented.

    b) Specify your career objectives as a professional social worker as you now conceive them. Indicate the fields of practice in which you are interested.

Note: the statement should not exceed two double-spaced pages

    The department should be notified immediately of any change of plans or address after the application is submitted.
